Policy Statement

 Incorporate a policy statement.  Here is a policy statement which derives from your Nondiscrimination Assurances:  "No person in the United States shall, on the grounds of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be otherwise subjected to discrimination under any program or activity, "for which the Recipient receives Federal financial assistance from DOT, including the Washington State Department of Transportation.”

Title VI Complaints

 Below are the procedures to be listed on your website, procedures which are listed in your Title VI Plan.  Also included in your Title VI Plan is a Title VI Compliant form (see also attached). Remove the current Title VI Complaint form and replace it with attached one.  Please remove this complaint procedures from your website and replace it them with below information and list the Title VI Coordinator contact information. Race, color, and national origin are the only protected groups under Title VI.

Any person or group who believes they were discriminated against based on race, color, or national origin in programs, services, or activities where there is federal assistance may file a Title VI complaint.

A complaint must be filed no later than 180 days after the date of the last instance of alleged discrimination, unless the time for filing is extended by the processing agency.

Complaints should be in writing, signed, and filed by mail, fax, in person, or e-mail. If a complainant phones a subrecipient with the allegations, subrecipients shall transcribe the allegations of the complaint as provided by phone and then send a written complaint to the complainant for correction and signature.
